From Fee-For-Service to Value-Based Care
TenderHeart began in 2004 as a typical fee-for-service (FFS) durable medical equipment company. In 2016, TenderHeart pivoted away from FFS to a value-based care model, specifically around incontinence. This model shifts incentives from providing a large number of low-quality products to providing the appropriate number of high-quality products. By 2022, the TenderHeart model had been adopted by all the national Medicaid managed care organizations (MCOs) in Texas. By 2024, the company was contracted with MCOs in Pennsylvania, Ohio, New Mexico, and Florida. Our reach continues to increase:

In September 2022, TenderHeart acquired Nextra Health. This acquisition combined Nextra’s well-respected name and industry expertise with TenderHeart’s innovative program offerings to better serve our patients and further our national reach. It also added Health Rewards member engagement programs to our service offerings. Our Health Rewards programs improve health outcomes for health plan members by providing evidence-based products to target areas such fall risk management, immunizations, diabetes, and many more.
