TenderHeart’s referral portal allows health plan service coordinators, physicians, and members to submit new referrals for durable medical equipment and check the status of existing referrals. To access the referral portal, click the icons below, or you can access the portal by clicking on the following link: https://referrals.tenderheart.com/
Submit New Referral
Click here to submit a new referral to TenderHeart Health Outcomes. A submission will generate an ID number, which you can use to check the status of a referral.
Check Referral Status
Click here to check the status of a perviously submitted referral. You will need the referral ID and member last name. If you do not have this information, you can view all of your referrals by clicking “referrals” in the main navigation bar and logging in.
